Hint: First you need to make out if the sentence is in active or passive voice. So, to understand the difference you can refer to the definition - A sentence is in the active voice when the subject performs an action and the sentence is in the passive voice when the subject receives an action.

Complete answer:
The above given sentence is in active voice, why? Well, because the subject is performing some kind of action. The basic rule that is in play while changing active into passive is that the object of the active voice sentence takes the place of the subject in the passive voice sentence.
Now, as we can see the above given sentence is in the formula –
Subject + verb + object + object complement, which will be changed into – new subject + verb + object complement + by + new object.
Now, let us look into the tense of the sentence, we have a simple past tense that will change the active form of ‘made’ into ‘was/were made’.
So, considering all the above given rules we can say that the correct statement to be formed is – he (it is the object pronoun ‘him’ in the original sentence) was made (simple past tense of made) leader by us (it is the subject pronoun ‘we’ in the original sentence.
Therefore, option a – he was made leader by us, is the correct answer.
Option b – the word maded does not exist
Option c – the verb made without the helping verb ‘was’ can not be used while changing the form
Option d – the object is missing due to which the meaning changes.

Note: Do not change the tense or the meaning of the sentence while transforming active into passive voice sentences. Many times similar looking options are given but one of them usually has some small mistake regarding tense or meaning of the original sentence.
